To register your child for the three or four-year-old Preschool/Head Start program, parents or guardians need to bring the following information:
A copy of the child's certified birth certificate
Social Security Card of applying child
Income verification--W-2 form or last 12 months pay stubs, previous year's tax return, Child Support, Student Financial Aid, Social Security, SSI, or K-Tap
